At Origins AFC, we proudly provide high-quality Adult Foster Care (AFC) services across Massachusetts, designed to support adults who cannot live independently due to medical, physical, cognitive, or mental health conditions. Our services are centered on compassion, respect, and individualized care in a safe and home-like environment.
Daily Living Assistance
We work closely with each participant, their family, and healthcare providers to create a personalized care plan tailored to their specific needs, preferences, and health goals. Our approach ensures dignity, comfort, and support in all aspects of daily life.
Individualized Care Planning
Our trained caregivers assist residents with essential activities of daily living (ADLs), including:
• Bathing and grooming
• Dressing
• Meal preparation and feeding assistance
• Medication reminders and administration
• Mobility and safety monitoring
• Personal hygiene and toileting
24/7 Supervision and Support
We provide constant care and oversight in a structured, supportive setting. Our goal is to
ensure each resident feels safe, valued, and empowered every day.
Healthcare Coordination
We facilitate communication with doctors, nurses, and therapists and help manage medical appointments, prescriptions, and treatment plans. Our staff works with a registered nurse who regularly monitors each resident’s health and progress.
Nutritional & Dietary Care
Healthy, balanced meals are planned and prepared in accordance with each individual’s dietary needs and cultural preferences. Special diets and feeding plans are followed closely.
Emotional & Social Support
Residents are encouraged to engage in social and recreational activities that promote emotional well-being and connection. We foster a warm, family-like atmosphere that supports inclusion and belonging.
Professional Oversight & Compliance
As a Massachusetts state-approved AFC provider, our program meets all regulatory and quality standards set forth by MassHealth and the Executive Office of Health and Human Services (EOHHS).
Our Staff

Qualified and Compassionate Caregivers
Our caregivers are carefully screened, trained, and committed to providing high-quality support in a family-like home environment. Each caregiver undergoes background checks, reference verification, and ongoing training to ensure they meet the standards set by MassHealth and the Executive Office of Health and Human Services (EOHHS).
Registered Nurses (RNs)
Every participant is assigned a Registered Nurse who conducts regular home visits, monitors health status, and provides clinical oversight. Our RNs collaborate closely with caregivers, primary care providers, and families to ensure the health and safety of each individual in our program.
Care Managers
Our Care Managers coordinate all aspects of service delivery, from creating individualized care plans to ensuring compliance with state guidelines. They serve as a vital link between participants, families, medical providers, and other support services.
Bilingual and Culturally Competent Staff
We proudly serve diverse communities across Massachusetts. Many members of our team are bilingual and trained in culturally sensitive care to better connect with and serve individuals from various backgrounds.
Ongoing Training and Professional Development
All staff participate in regular training in areas such as:
- Medication administration
- CPR and First Aid
- Behavioral health support
- Infection control and hygiene
- Dementia and Alzheimer’s care
