AFC Program
Adult Foster/Family Care (AFC) is a state-funded program for individuals living with a caregiver that receive physical hands-on assistance or supervision/cuing with Activities of Daily Living. The AFC is responsible for oversight and compensating caregivers to care for individuals in a home-setting.
All of our AFC Provider staff will encourage & provide opportunities for our members to maintain/increase their social contacts, personal relationships & community network.
AFC Staff will discuss preferences and desires with the member. Encouragement will be provided to seek out & attend community programs, visit friends & family, and interact with their communities.
AFC Staff will provide information to our members on various opportunities in the community, including but not limited to educational sessions, elder service programs, visits to community settings such as parks, musical events, libraries, etc.
Who Pays For Your Care?
Medicaid, MassHealth, Medicare and most other insurance programs can be
billed for services based on determined insurance criteria.
Who Is Eligible For Adult Foster Care?
Your loved one must:
- Be 16 or older
- Be unable to live alone due to a physical, medical, or cognitive disability
- Be eligible for MassHealth
- Require assistance with one or more activities of daily living (ADLs)
You Must:
- Live in Massachusetts with the person in need of care
- Have adequate time to provide personal care and safety
- Not be the loved one’s legal guardian or spouse
- Complete a screening and application process
